Nursing Services

Our nursing team provided comprehensive patient care from admission to discharge. This care includes:

  • Initial Patient Assessment including nutritional screening
  • Comprehensive patient education
  • Placement of intravenous catheters (peripheral, midline and PICC)
  • Management of Intravenous catheters
  • Performing skilled services such as administering medications, dressing changes, blood draws
  • Communicating with the members of the healthcare team regarding identified care needs and response to therapy

Pharmacy Services

Our pharmacy staff provides clinical consultation and monitoring throughout your care that includes:

  • Communicating with the physician to obtain the necessary prescriptions and develop a treatment plan
  • Compounding sterile IV products following industry standards in our modern, state-of-the-art pharmacy
  • Assessing the therapeutic appropriateness of the prescribed drug regimen
  • Evaluating for therapeutic duplications, potential drug-drug and drug-nutrient interactions and incompatabilities
  • Monitoring the patient's supply needs and ordering additional supplies as necessary
  • Scheduling deliveries both routine and emergency
  • Managing the disposal of medical waste

Patient Focus

Patient Care Services/Delivery

American Outcomes Management of New York has extended regular office and delivery hours to accommodate the patient's personal schedule. The office hours are from 8:00 a.m. to 6:00 p.m. Monday through Friday. Patients have access to our 24-hour toll free phone number. A nurse, pharmacist and delivery technician are available round-the-clock to meet the patient's care needs.

Comprehensive Patient Education and Training

American Outcomes Management of New York strives to foster independence in the patient and caregiver in administering prescribed therapies and using specialized equipment. Patient education begins at the time that the referral is received. The course of training and education is based upon the patient's individual needs. Each patient receives a customized training packet with therapy specific educational resources. The pharmacist and nurse work collaboratively to provide the patient with comprehensive information regarding medications, AOM, supplies and equipment. A Certificate of Education and Training is completed to document patient/caregiver competency with all necessary skills required to provide safe care in the home. The nurse monitors compliance on each subsequent visit and reinforces the education process.

Patient Care Management

Each patient is assigned a dedicated clinical and operational team who are responsible for assuring the quality and cost-effectiveness of the patient's care needs. The team is led by a Patient Care Manager who is responsible for coordinating all of the patient's services from admission to discharge. The Patient Care Manager communicates on a regular basis with the patient, physician, pharmacist and other members of the healthcare team. The Patient Care Manager also contacts the case manager to keep them apprised of the patient's clinical condition and ongoing care needs and to obtain the necessary authorizations for service.
